Health care professions are becoming acutely aware that health care demands a specific and unique endeavour; mere disease intervention does not suffice. The consumer is furthermore currently seeking self determination at the health pole of the health-disease continuum. This paper proposes the use of a questionnaire which has been produced as a health tool to facilitate practitioners in their role as educators at the consumer-health care system interface.

Project participants recognised this questionnaire as a health tool which enhanced awareness of certain relevant health behaviours, provided a strategy for health promotion and included a self-monitoring format. Results suggested that the questionnaire should not be used alone. It is anticipated that the best results will be achieved when the questionnaire is supplemented by verbal practitioner communication.
